CONCLUSIONS

One hundred and three surgeons from 29 countries in 5 continents participated in the first Hypospadias World Congress of the Hypospadias International Society (HIS) in Moscow, 30.08-01.09.2017. They were from USA, Germany, UK, Austria, France, Russia, Holland, Poland, Hungary, Romania, Greece, Mexico, Turkey, Egypt, Emirates, Palestine, Saudi Arabia, Morocco, India, Pakistan, Indonesia, Taiwan, Kazakhstan, Ukraine, Azerbaijan, Uzbekistan, Armenia, Georgia and Moldavia. There were 63 presentations, 2 panel discussions and 4 live operations. They have reached the following conclusions:

Conclusions of Moscow Hypospadias World Congress, 2017

  1.  Hypospadias is a common anomaly with incidence of 1 in 125 live male births. The incidence may be increasing in some parts of the world. Epidemiological studies and etiological factors are areas of research which will be co-ordinated by the HIS.
  2. The normal development of the human penis and urethra and the pathogenesis of hypospadias are still unknown and scientific research has to be initiated and encouraged to find and document the embryology of the human urethra and pathogenesis of hypospadias. Collaborative studies between centers can provide embryological material and laboratories can be streamlined by the HIS.
  3. The Hypospadias International Society (HIS) has dedicated a prize of 5000 € to the best scientific research that will document the embryology of the urethra or pathogenesis of hypospadias. A scientific committee designated by the HIS will assess the short listed researches.
  4.  There is a need to reach an international classification and assessment tool of hypospadias that will help to standardise the different grades of hypospadias and to perform multi-centre studies among different countries all over the world.
  5. The Hypospadias International Society (HIS) has allocated a team of 7 Professors from Germany, USA, France, Austria, Turkey, Russia and Pakistan to reach an International classification until the next meeting in October 2018. Members are encouraged to discuss their classification proposal with other surgeons by opening discussions at local meetings ( e.g. in Alexandria, Istanbul).
  6. Although there are 2-300 hypospadias publications each year, the lack of long term follow up and small patient´s number limit our ability to make high impact conclusions based on these studies).
  7. The current popular techniques for correction of Distal Hypospadias have long term complication rates as high as 30%, which contrasts with what is being published in literature. There is a need to stress the importance of long term follow up when evaluating different techniques.
  8. The Hypospadias International Society has allocated an annual prize of 1000€ for the best clinical study thanks to the kind donation by Professor Moneer Hanna.
  9. Hypospadias is a complex and challenging anomaly with wide spectrum and requires specialisation, dedication and long term follow up.
  10. The Hypospadias International Society (HIS) recommends to refer hypospadias patients to referral centres which perform more than 50 hypospadias operations every year with good results and arrange for long term follow up. Centers who reach these numbers are invited to become institutional members, with concessions to its staff.
  11. The Hypospadias International Society aims to establish an interactive website, electronic Journal and workshops all over the world to upgrade the quality of service for children and adults with hypospadias.
  12. The second Hypospadias World Congress will be held in Frankfurt, Germany in the period from 3-5 October 2018.
